Summit Prime FIA Health Waiver Endorsement

Summit Prime FIA policies currently include a Health Waiver Endorsement. This Endorsement is for both Non-Qualified and Qualified Funds but may not be available in every state.

These provisions include:/p>

Terminal Illness - the Annuitant has a non-correctable medical condition that is expected to result in death within twelve (12) months of the date that the condition was first diagnosed by a Physician.

Disability - the Annuitant qualifies for disability income from an insurance contract, from an employer’s benefit program, or from a governmental agency.

Home Health Care - a Physician’s diagnosis that the:

  1. Annuitant has been continuously unable to perform at least two (2) of the six (6) Activities of Daily Living without the physical assistance or presence (within arm’s reach) of another person, to prevent, by physical intervention, the Annuitant’s injury;
  2. Annuitant’s limitations are not temporary; and
  3. Annuitant’s limitations have existed for at least ninety (90) consecutive days.
